• Family planning services: 90%

• Health homes: 90%

• Expansion Medicaid: 90%

• District of Columbia: 70%

• U.S. territories: capped at 55%

• Most administrative services: 50%

• Smoking cessation for pregnant women: FMAP +1%

• Specified preventive services and immunizations without cost-sharing: FMAP +1%
