Aircraft accidents due to air traffic control error are an extremely rare event, but they have happened. Such was the case in late Spring in 2004 when a Piper PA-44 Seminole impacted mountainous terrain in Southern California at nightafter a radar air traffic controller at the SoCal TRACON used incorrect phraseology when issuing an altitude assignment. Based on this aircraft accident event, do you think the SoCal TRACON controller was entirely at fault for this accident? Or was the flight crew in the accident aircraft fully responsible? Or... was it a combination of both the controller and the flight crew?
