Community Health Improvement Plan and Hospitals Response: Every hospital is required to develop a Community Health Improvement Plan which shows how they will respond to the top issues and concerns affecting the population in its county. The County Health Department in collaboration with the hospitals located in the county as well as other key stakeholders, develops a Community Health Needs Assessment CHNA This is what the hospital will use as well as other resources and data, to develop their Community Health Needs Assessment since their service area is larger than just one county.
