Case for ethical analysis: You are on a team that has been providing care, including annual wellness exams for Ms. Angel, a 25-year-old female, for the past two years. When she was establishing care with you two years ago, you recommended an HIV test as per the current Centers for Disease Control. She declined testing at that time, stating that she did not feel that she was at risk of the disease. The following year, you asked again about HIV testing, and she again declined. She presented last week with amenorrhea and fatigue. A urine HCG was positive. At that point, you once again revisit the discussion about HIV testing as part of routine care during pregnancy. She initially declined but upon further discussion, she told you that she is scared because as a teenager, she used injection drugs and engaged in other behavior that increased her risk for HIV and other infectious diseases. She shared that she is proud to be in recovery, proud of where she is in her life. She is in a committed relationship but hasn't told her partner about her past because she is ashamed. After a heartfelt discussion, Ms. Angel agreed to get screened for HIV as well as hepatitis B and C. Unfortunately, screening tests for both HIV and hepatitis C are positive. She is here today to follow up on her results. When you share them with her, she is devastated. She vehemently states that she cannot share this information with anyone in her family, including her partner. |
