Grace is a 40-year-old Caucasian female who has been referred for drug use treatment by the court. This is her self-report. She was arrested for possession of Xanax, Valium, and Ambien which she had bought on the street from an undercover police officer. This is her first offense so she was sent to a diversion program. Grace began experimenting with alcohol and marijuana when she was 17. She drank or smoked when she went to parties but did not use other than then. She began to work as a flight attendant immediately after graduating from high school. She continued her pattern of drinking 2-3 beers at a party and smoking a few hits from a pipe as well. She met her husband, Greg, when she was 24. They were married and decided together that they did not want children. Greg was an attorney and he asked her to stop working since she was gone as a flight attendant several nights a month. Grace agreed and began to keep the accounts for Greg's practice. Greg was killed in a drive-by shooting 2 years ago. Grace held him while he died and she screamed for help. Grace withdrew into a depression, experienced panic attacks, and had nightmares of the shooting. She went to her doctor who prescribed Ambien for her nightmares and Xanax for her anxiety. She took the medications but they only helped for a short time. She began to take more than was prescribed and drank alcohol with them to "drown my memories". Six months ago, her doctor refused to continue to write her prescriptions if she did not begin therapy so Grace began buying pills from people she had met when trying to find another source of supply. She was taking 10 Xanax and/or Valium, drinking a bottle of wine, and taking 4 Ambien to sleep every day. She states that she knows she is addicted but cannot stop since as soon as she does, she feels the depression and anxiety again and the nightmares return.

In the first session Grace cries heavily when she talks about seeing her husband die. She states "there was so much blood. I couldn't help him. He died in my arms and every time I try to sleep without all of the Ambien, I wake up screaming that his blood is all over me. I know I shouldn't take so many pills but I just can't function at all without them. I need help but I need it for the trauma much more than the drug use."
