Examine your organizational structure to see what might be causing these problems. Some of the problems could be coming from the management not listening and caring for their employees. They aren't enforcing standards and the good employees are getting tired of the lazy employees getting paid for not doing their jobs. This shows because the talented employees are leaving and they aren't treating the employees equal in the company with showing favoritism.
What kinds of design choices do you need to make to solve them? I would post and start enforcing regulations in the work place visibility for all company employees with mandatory training for all employees to include management training for all managers, training on ethics, time management, equal opportunity, leadership training, reward recommendations, work place safety, and communication in the workplace. This would show the employees the organization cares about the workplace and their employees
