Workers from the daily operations team and maintenance department at ETILENO petrochemical plant (top tier COMAH site) are planning maintenance for valves V-11 and V-12. In order to conduct the job, they have to isolate a section of pipe to safely remove V-11 and V-12 and send them for servicing. You have been tasked to write an operating procedure (as you have learned in the Pilot Plant modules EG-220, more specifically in EA1 sessions, and EG-230), also known as SOP (standard operating procedure), to perform an isolation of a pipe section and then remove valves V-11 and V-12 from the pipeline (Stream 3), taking into account all safety implications involved in this maintenance job. Remember that workers will follow your operating procedure. Therefore, the correct order or sequence of the steps is crucial as this will be part of a safety critical procedure. Unclear or missing steps, wrong order of the steps/instructions can have serious consequences in industry, and accidental loss of containment can escalate quickly to a major hazardous event in this case. This work must be carried out somewhere in the transferring lines (located outdoors) where liquefied Ethylene is sent from Cavity 1 and Cavity 2 (underground storage), to one of the Ethylene Spheres on site. The diagram in Figure Q1 shows the valve configuration in all transferring lines (Stream 1, Stream 2 and Stream 3). The operating pressure in all streams is 80 psig, temperature -10 °C. System should not fall below 50 psig, otherwise it will cause pipework overchilling and brittle fractures. Pipelines (4") will be able to withstand more than 80 psig (design pressure 200 psig). >99% Nitrogen in the pipeline must be achieved in order to complete the isolation and maintenance job. (DATA-Ethylene Flash Point: -136°C). Decide and state in your answer which pipe section you will perform the isolation on to remove V-11 and V-12, and write the operating procedure for this task to be carried out (number all steps/instructions). Include in your answer all the checks (lab tests, pressure, etc.) that are required to be done, and indicate in the procedure where Lock- Out Tag-Out can be applied Indicate steps that are safety critical and explain the reason why. Your answer will be assessed based on the following aspects: logic [10 marks], steps/instructions [15 marks], lock-out tag-out (5 marks], and explanation of safety critical steps [10 marks].
