My Personal Mission Statement: To protect health, improve lives, and promote a healthier quality of life.
What am I trying to accomplish? To improve the quality of care we provide and influence healthier living.
What are the processes or activities involved in the response to a? Ongoing evaluation of training tools and assessing the effectiveness of health education given to the patients.
What is the purpose of the response to b? In public health, we have to address the social determinants of health and what inhibits families from achieving health and sustaining it long-term.
What is the purpose of the response to c? Address any limitations to accessing health care services and focus on any barriers preventing health promotion. Using a patient survey to collect data about active barriers.
What is the purpose of the response to d? To better understand specific factors that are preventing health maintenance and develop a strategic plan of intervention and sustainability for those families who are interested in making the necessary changes. Also, establishing an outreach plan to loop those families who have not bought into the idea of health promotion.
What is your purpose according to your patients, employer, or someone else? To improve patient health through education and community outreach, and providing resources for health care access and access to healthier food options. Task the providers to enrich health education by addressing health disparities that are culturally applicable and making sure the information presented is easily understood by the patients. For the employer, improved patient outcomes warrant more federal funding to continue addressing health disparities in the community.
What larger purpose may eliminate the need to achieve this smaller purpose altogether? Patients seeking healthcare options outside of public health services.
