Clearly state the problem of practice you have chosen to address and include the evidence you collected to validate the problem and the root causes you identified. Articulate the aim of your project. Description of Initiative Provide narrative that describes the primary drivers and change ideas you selected for your initiative, including relevant research that informed your decisions. Describe any major changes/adjustments you made throughout the process and incorporate your progress monitoring data (PDSA cycle, formative measures, etc.) for each. Findings Include analysis of your measurement data to evaluate your improvement work. Use tables and figures when appropriate. Recommendations Describe recommended next steps based upon your results
