1. Record in your contact notes a termination interview, indicating the reason for termination and any follow-up arrangements you made for the client after she leaves your services. Be sure your termination interview includes the parents, and indicate if the child was present. Refer to the goals for such an interview in this chapter.

2. Write your child’s parents a letter as a follow-up to this interview, summarizing for them the discussion and resolutions that took place in your last contact.

3. Prepare a discharge summary on this child. It will be the last item in the client’s chart. Use the guidelines for a discharge summary found in this chapter.
