Complete living will/advance directive paperwork: Talk with the person you choose to be your health care
Question:
Complete living will/advance directive paperwork:
• Talk with the person you choose to be your health care proxy.
• Reflect on your experience. You can use a hypothetical proxy to talk to as part of this activity.
1: You are not required to sign the advance directive paperwork you complete nor will you need to turn it in as part of this assignment. You may even want to destroy the paperwork after you complete the exercise if you not wish to have a living will/DPA. You should, however, complete the paperwork and use it as a guide when talking with your proxy.
2: If you already have advance directives, you should still watch the videos and complete the paperwork found on the Caring Connections web site. You may want to name a different person as a hypothetical proxy and have the conversation with him/her or if you have changed your mind about an aspect of end-of-life care you can have a conversation with your official proxy to discuss this change.
