Each survey is created by one or more Designers. Each survey consists of one or more Questions, and each Question can be of the following types: Open-ended (or fill-in-the-blank), True-False, or Multiple Choice. Each Survey/Questionnaire is administered by one Administrator. The Administrators set up the Questionnaire and invite many Participants to participate in the questionnaire. To distinguish participants, we want to track their name, address and job title.
