A hospital database is designed to keep track of doctors $($identified by doctor$\_$id$,$ with specialization and email as attributes$),$ patients $($identified by patient$\_$id$,$ with name and date$\_$of$\_$birth as attributes$),$ and treatments administered by doctors to patients $($with treatment$\_$date and treatment$\_$type as attributes$).$ Doctors can be assigned to multiple patients; each patient has a primary care doctor $($who is one of the doctors$)$; and each treatment must be uniquely identified by its treatment$\_$date when the doctor and patient are known. We are not interested in the treatments given by a doctor once they leave the hospital. Additionally, doctors collaborate on research projects $($identified by project$\_$id$,$ with project$\_$name and start$\_$year as attributes$),$ and each project must be uniquely identified by its project$\_$name when the lead doctor is known.
