6. Draw up an appropriate implementation/ action plan and transition plan for this improvement. The plan should describe the improvement, nominate the resources that will be required, detail the personnel to be involved and indicate milestones and deadlines. Charts and graphs might be appropriate. The description of the intended improvement should be formatted in such a way that it can actually be presented to senior management of the organisation as a continuous improvement proposal
