Scenario: You are the Director of Health Information Management (HIM) for University Hospital (UH), a 500bed teaching/research facility which includes multiple satellite sites around the region. You have been charged by the Chief Financial Ofcer (CFO) with putting together a team for implementing computer-assisted coding (CAC) at UH. The CFO expects the implementation plan (not the computer- assisted coding project itself) to be in place Within the next two months. You will be the project (team) leader for this project. There are multiple electronic systems in place that include inpatient and outpatient electronic health records. The HIM electronic systems are technically managed by Information Services (IS) Application Specialists. All services for coding and maintaining records for the hospital and regional satellites are centrally located at the base hospital. The HIM Department codes all inpatient discharges, outpatient surgeries, and observation stays for the hospital, clinics, and satellite sites. There are 20 AHIMAcredentialed coders. The HIM Department utilizes the 3M electronic encoder for coding. Many of the attending physicians are employed by the hospital; the Professional Billing Division (not HIM) oversees the physician- services coders. Emergency Room coding is done by an outsourced coding company. No coding is performed by Patient Financial Services (PF S) but the electronic billing systems ensure that coding is present on all claims that are billed by PFS. There is a Decision Support Department that pulls all the data from the data warehouse for reporting purposes both externally and internally. An Information Services Data Reporting Team works closely with Decision Support to ensure that the data is being pulled correctly from the electronic systems. The Research Division extrapolates de-identifled data, including coding, from the data warehouse on a weekly basis. Coded data also is shared with the Ohio Hospital Association, Greater Cincinnati Hospital Association, HealthBridge (Regional Health Information Exchange), and the Ohio Department of Health. Patients and family have access to coded data via a patient portal; their admission/discharge diagnoses and procedures are available for viewing
