Under the Cup Agreements (CAs), CH gives free urine specimen cups to the physician as long as these certain conditions are met: (a) The physician must "always" order the CHs "Custom Panel" that tests for all CH offerings "regardless of whether the patient needs" the testing or not; (2) Upon the need for additional and required testing, the physician must refer back to CH for those additional tests; and, (c) If these requirements are not complied with by the physician, CH will charge the physician/provider for the cups.
