Activity 18: Review effectiveness services

In consultation with your Workplace Supervisor identify one client that you can work with to review how effective a service or program they are accessing is. Try to ensure this is a client you have supported, either directly or indirectly (by been involved with supporting a staff member or supervisor).

Interview the client, with the support of a co-worker, relevant supervisor or your Workplace Supervisor. Ask the questions in the template provided on the page that follows and capture the client's responses or feedback. Alternatively, you may use any standard form the organisation typically uses during the service review process to capture client feedback.

Be sure to document feedback constructively and professionally.
