An operator at a subassembly process in your factory takes 14 different items, assembles them together and then closes them in a housing. Once closed, you cannot see the assembly inside the housing. An FMEA on the process resulted in a high RPN (risk priority number) at that process step because an incomplete assembly could be contained in the housing and shipped out to customers.
