As you think about risk for projects, how extensive do you need to go in documenting risks? Should you stop at risks that would only cause a significant delay or cost? Or should you work to document all risks that you can identify for a project?

Also, for risks that you have identified, how far should you go in creating mitigation plans to avoid or minimize the risk? Should every risk have a detailed mitigation plan or would you only focus in on the critical ones?
