Assessment Plan $$ AH $290$

As your project draws to a close you will want to think about assessing your plan.

When you think about your plan, consider how you will assess the plan and how

you will know if it is a positive or negative outcome. In addition, what changes do

you have in place if you need to make changes in any form of the plan.

You should have a $3-$month, $1$ year and $3-$year plan. It does not have to be more

than one page, but it can be as little as $1$ paragraph or steps$/$bullet points if that

works better for you.

This could be a follow$-$up survey for the participants and the staff. Listen to

suggestions from the individuals doing the work. Be sure to think about what

went well and what didn$$t go as planned. Brainstorm with others on best

practices.
