Assignment: Create Fishbone Diagrams + 5 Why Diagrams for each Incident

Details for Incident 1 On Tuesday, July 15 at 1:45 pm In Parking Lot 24A, a worker driving a van to deliver packages across campus struck another vehicle while backing up. Nobody was injured but there were large scratches on both vehicles (Rear passenger side of van, rear driver side of Vehicle #2) that would not interfere with their operations. Interviews were conducted by the driver of the van and a bystander/witness. The vehicle struck was unoccupied at the time. Upon interview, the driver indicated that he was distracted by a phone conversation. He was using a hands-free device, where he was conversing with his supervisor via speakerphone about an item that was just dropped off. It had been addressed incorrectly and needed to be picked up before his next stop so that it could still be delivered on this run and prevent extra trips or wasted trips. He was also in a hurry because they were already behind schedule for the day. He did not see that the vehicle to his right did not pull into its space straight, so he contacted it while backing up. He had not been drinking and doesnt use drugs (drug screen 4 months ago was negative). This is a fairly new employee, started work 3.5 months ago. He normally uses a different vehicle to make deliveries, but it was being serviced that day. Driver also stated that he had been tired, sometimes gets drowsy right after lunch. The bystander indicated that the driver didnt look around much when he was backing up and did not notice that the vehicle beside him was parked at an angle. This vehicle had its rear driver-side bumper slightly crossing the boundary line for the parking spot and into the van drivers parking area. The bystander stopped walking to make sure he wasnt in the drivers way. During the backup, the rear passenger side of the van scraped the drivers side rear bumper on its way out. He also thought the driver backed out too quickly (gunned it a little too much).
