In April 2018, there was a failure of an IT project in the United Kingdom namely an online banking disruption by TSB Bank. The incident occurred as the bank moved customers to a new IT platform where the platform was not fully tested. According to the audit, which was commissioned by the bank and run by law firms Slaughter and May, the board failed to ask important questions to its contractors before launch, resulting in about 1.9 million customers being locked out of their accounts. In the weeks following the failed launch, millions of customers experienced major disruptions online and in-store. Slaughter and May have criticized both TSB and IT vendor Sabis in an independent review of the incident. Customers have been moved to the new system, but according to the report, it was not thoroughly reviewed before being broadcast live. It has been found that the tests are conducted only in a controlled environment and not in a real -world environment. According to the report, TSB acknowledged that if testing had been conducted on both systems, it might have been able to identify vulnerabilities that endangered customers before they occurred.
