Assignment #$3$ Questionnaire Creation

Objective:

The objective of this assignment is to assess student$$s understanding of questionnaire design principles,

scale types, and scaling techniques, while also allowing students to apply this knowledge in a practical

context within a chosen organizational department.

Instructions:

$1)$ Pick one department within an organization which you are interested in working for $($i$.$e$.,$ Human

Resources, Finance, IT$,$ Marketing, Sales, etc.$)$

$2)$ Generate a research question relevant to the chosen department, focusing on the formulation

of a questionnaire to collect primary data through employee responses.

$3)$ Develop a questionnaire $(10$ question minimum$)$ that could be distributed to employees within

the organization.

$$ Generate your questionnaire using MS Forms

$$ Create an introduction for your questionnaire that includes the department running the

survey, and the research question. Be sure to also include the remaining elements of an

introduction we discussed in class!

$$ Ensure you have considered all elements of questionnaire design $($i$.$e$.,$ form of response,

question sequence, question wording, etc.$)$

$$ Be sure to include at least one question with a nominal scale, one question with an

ordinal scale, one question with an interval scale, and one question with a ratio scale.

$$ Attempt to use a variety of scaling techniques discussed in class for the

nominal and ordinal scale questions.

$$ Create a PDF print of the questionnaire in full.

$4)$ For each question on your questionnaire:

$$ Indicate the scale type $($nominal$,$ ordinal, interval, or ratio$)$

$$ For nominal$/$ordinal questions $$ indicate the scaling technique used $($i$.$e$.,$ dichotomous,

category, numerical, etc.$)$

Note: This can be completed as a separate word document listing each question number $($for

example $1.$ Ratio, $2.$ Nominal $$ Dichotomous, etc.$)$ or you can directly mark up your pdf of the

questionnaire with this information.

Deliverable:

PDF print of Questionnaire $(10$ questions minimum$)$ taken from MS Forms $($or similar software$).$

Summary of scale type$/$techniques used for each question $$ as either a separate MS Word document

or as a mark$-$up on your PDF print of the questionnaire.

Submit the assignment in the Assignment drop box, before the due date. Submissions more than $5$

minutes late will be penalized $10\%.$ Assignments received after that WILL BE GRADED AS ZERO.
