Can this implementation plan statement have fewer words? We are developing a high-level implementation plan to enhance healthcare in nursing homes. This involves gathering input from residents, families, and staff to assess health needs and current services. We have set specific, measurable objectives, such as reducing hospitalization rates, and have developed key performance indicators to track progress. In selected units before the expansion, we have set up a pilot program and follow-up assessments to monitor resident health and satisfaction. Our exit strategy includes evaluating the success and effectiveness of sustainable health practices to determine if they can be sustained without external support. We will create and reallocate resources to ensure that foundational care continues, even if funding is reduced, and provide strategies and support for residents during the transition
