Case Study 1 - Company A Hazard Assessment [10\%] - complete in 2's or 3's You are working for one of the fast food locations in Prince George, B.C. as a safety inspector let's call it Company A. Bob is currently on the workstation that requires him to feed sandwiches through a grilling machine as shown in the image on the left below. Sandwiches travel from right to left on this machine while they are being heated and/or grilled as they travel through the machine/oven. The red toggle power button (shown in expanded view next to the oven) on the top left front of the machine has a light in it so that when the button is toggled on the light comes on indicating that the machine is on and ready to use. This machine sits on a separate small stainless bench in the middle of the kitchen as shown in the next diagram. The red arrow indicates that the machine is on the left side of the bench with the red arrow indicating the direction of travel of a sandwich. The sandwich Bob is working on producing for the customer gets stuck half way through the machine. Bob shuts off the machine using the red power / toggle button and reaches his hand in to attempt to free the sandwich. As he reaches in, his body contacts the red rocker switch turning the power back on. His shirt sleeve is caught in the conveyor apparatus which starts moving while his hand is being burnt at the same time. Bob screams and a coworker, Sue, rushes to turn off the machine via the red power button and helps to free Bob's arm/hand from the machine. You are in another part of the kitchen and the restaurant owner asks you as the safety officer to investigate what has happened. As part of your investigation you are asked to list the following: A) The unsafe acts that were encountered during this activity. B) The unsafe conditions during this activity. C) The types of energies involved. D) The steps you would recommend to be taken to prevent this situation from occurring again
