Introduction
Norman had his taxes prepared by an H&R Block tax preparer several years ago, but for the last few years, he has prepared his own taxes using a competitor's software. He believes his return is more complicated this year, and he decided to seek professional assistance. He was also happy with the service he received in September 2021, after he received a notice from his state. An H&R Block preseason Tax Professional helped him resolve the issue.

Norman paid $1,150 per month to Happy Acres to cover the rest of his father's rent. Happy Acres is not a skilled nursing facility, and Maxx does not live there because he requires medical or nursing attention. Other Income Information
Canceled Debt
Norman purchased a 2018 pickup truck on October 18, 2018, for $33,457 for his personal use. Unfortunately, he was not able to keep up with the payments on his pickup. He did not refinance the loan prior to the cancellation of debt, and he was not eligible for any pandemic-related accommodations. The vehicle was repossessed on February 3, 2021. He received a Form 1099-C.
At the time of the cancellation, the balance of his loan was $28,752, and the fair market value (FMV) of the truck was $17,463.15. His other assets consisted of his home, purchased for $230,157 and valued at $215,476, with a loan balance of $210,577. His credit card balance was $4,753. The value of his 401(k) is $457. He had almost depleted his savings account, but he did have a balance of $1,500 in the account immediately before the debt was discharged. The value of his stock portfolio was $10,943. Norman also has an outstanding balance on a student loan of $18,508. The furnishings in the home were purchased for $23,850 over the years and are now valued at $7,980. The basis in the home and furnishings did not change from the discharge date to the beginning of the year after the year of the discharge.
You may use the Canceled Debt Worksheet to determine Norman's insolvency, which will help you determine the taxable amount of the canceled debt.
Put Option
Norman purchased a put option for $300 on March 18, 2021. The option gave him the right to sell 200 shares of BGD stock for $50 a share. The option expired unexercised on June 18, 2021.
Incentive Stock Option
Over the past couple of years, Norman has been granted incentive stock options (ISOs) by his employer, Open Road Freight. He was granted the first option on October 1, 2019. This option allowed him to buy 300 shares of the company's stock at $33 per share. On February 3, 2020, Norman exercised his option, meaning that he purchased 300 shares of the stock at the grant price of $33 per share. The fair market value of the stock at the time was $35 per share. In January 2021, Norman received a Form 3921 from Open Road Freight reporting this purchase. He thought this form might be important, so he kept a copy of it. On March 5, 2021, Norman sold the shares he had acquired when he exercised that option.
Norman's employer granted a second incentive stock option on June 1, 2020. This option allowed him to buy 100 shares of the company's stock at $40 per share. Norman exercised this second option on November 2, 2021, when the fair market value of the stock was $50 per share. Norman has not yet sold any of these shares, but he did receive another Form 3921 in January 2022 reporting the purchase.
Gambling
Norman is not a professional gambler, but he enjoys playing slot machines every so often. He had some winnings and losses this year. He followed advice he received last year to document all his gambling expenses, and he provides a copy of the ledger sheet shown below.

Information Documents
Norman had income from several sources. He provides a Form W-2 reporting income from wages. He also provides a Form 1099-INT reporting interest income, although he had to pay an early withdrawal penalty when he withdrew funds from a certificate of deposit before it matured. In addition, Norman purchased a put option which expired, and he sold shares of stock through an employee stock option plan. He provides two Forms 1099-B and two Forms 3921. Norman also won some money playing slot machines, and he provides two Forms W-2G. Some of Norman's winnings were for smaller amounts for which he did not receive a Form W-2G, but he provides a document listing all winnings and losses. Norman's only other income consisted of income from cancellation of debt, and he provides a Form 1099-C he received from the creditor. Finally, Norman provides a Form 1098 he received from his mortgage lender.
Norman did not provide a Form 1095-B or 1095-C, but as noted above, he tells you that he had qualifying health care coverage through his employer all year. He states that his father was covered under medicare.
