Client First Initial: P Age: 2 Gender: F Primary Diagnosis: Autism Spectrum Disorder Past Medical History: Paidyn is a 2-year-old female diagnosed with autism spectrum disorder (ASD) within the last year. Paidyn has presented with developmental delays in early childhood and has been receiving occupational therapy services for the last six months. Per mother's report, Paidyn was born with the umbilical cord wrapped around her neck causing a brief NICU stay following birth, however, no complications or oxygen required. Mother reports no other complications and no past surgeries or other significant medical history. Precautions: None indicated Reason for OT referral: Paidyn was referred to occupational therapy for delays in fine and gross motor development as well as sensory processing challenges
