Physicians who work at a medical center see many patients. The center keeps each physicians personal information including id number, title, first name, and last name. The center also keeps each patents id and her first and last name. Each patient may be seen by more than one physician, although on different appointments. During an appointment with a physician, which occurs on a particular date and time, a patient may be diagnosed with one or more illnesses associated with the degree of each illness. An illness has its unique medical name and common name that can be diagnosed in different appointments for different patients.
Hint: One medicine can be prescribed in many appointments; and in an appointment, many medicines may be prescribed (many to many relationship). Therefore, you may need an associative entity.
