Could someone help me write up a learning contract just for reference? It can be for any organization preferably healthcare related. This is the template my university uses.

Capstone Description: In 4-7 sentences, describe where, why, and how you will conduct your Capstone Experience/Project. Briefly describe the agency or setting of your project, and why you chose to address an issue in that particular agency or setting. List the various activities and resources you plan to use to accomplish your goal. Program Wide Goals: Your contract objectives and measures should provide evidence that you met these four HSA Program Goals: 1. Develop the knowledge, skills, attitudes and values necessary for positions of responsibility and leadership in the continually changing health care or human services profession. 2. Provide advanced professional and educational services applying the appropriate critical thinking, interpersonal, and written communication skills required for successful performance in the health care or human services professions 3. Use technology to acquire knowledge, aggregate information and communicate effectively to others. 4. Apply the principles of sound research design to the critical evaluation of scientific literature related to the health care or human service professions.
