A welding shop has been having some problems lately with an electric weld they are doing on a stainless steel fitting they make for a company down the street. The supervisor calls a meeting and gets everyone to brainstorm what may be the problem. Put the following ideas onto a Cause and Effect diagram with the main branches of manpower, machine, material, method, measurement and mother nature/environment.

The welding rods stored just outside the locker room showers make lousy welds due to condensation settling on rods from open locker room doors

The instructions posted on the welder are for plate steel temperatures, not stainless steel

The power dial on the welder is loose and falling off

Half the workers have not been trained on the new machines

The workers are modifying some of their tools

Bulbs are missing from many of the overhead lights

The old x-ray machine for checking welds takes lousy pictures

The quality of the stainless steel varies between suppliers

The last two cases of rods came in tins that had the seals broken and seem to be mixed types

The hardness testing device is broken

The batch of rods in storage area have different melts mixed together

When following the standard operating procedure, part of the weld cannot be viewed by the operator
