Create a process flow chart for an outpatient clinic visit, urgent care, or dental visit. Include the patient flow from check in to check out. Identify the points in the process where you decrease risk of harm, breech of privacy, and/or dissatisfaction. You may observe at a local office/UC, interview, or a mapping of an experience you have had. Extra points given to those who interact with the local community in any area of medicine/health care rather than using what you think would happen.

• Create and submit a process flow chart diagram of the process:
  • Define the basic stages of a process then further define the process into specific steps. Make sure you get into the detail of steps (potential examples: patient movement, staff involved, tests completed).
  • Use traditional flow-charting shapes to represent different steps or actions.
  • Depict where realized or potential gaps exist.
• Write a summary (minimum 500 words):
  • How effective is the process in meeting customer requirements?
  • Are there performance gaps or perceived opportunities for improvement?
  • Have the relevant stages of the process been represented? Are owners of each stage represented on the team? If not, what needs to be done to gather their feedback and ideas?
  • What are the inputs required for the process and where do they come from? Are the inputs constraining the process or not? Which ones?
  • Are there equipment or regulatory constraints forcing this approach? Variable(s) that maybe impacting the identified gap?
  • Is this the right problem-process to be working on? To continue working on?
  • What would you set a goal for the time between arrival and being called back to see a provider?
  • Are there any steps that are missing?
  • Apply what you have learned in the book and literature reviews (both in class assigned and research on your own).

Include a copy of your process flow chart and your written summary. Include at least two (2) peer-reviewed articles. APA formatting required.
