REQUIRED FORMAT

I. COVER
A. There should be a color photo cover.
II. TABLE OF CONTENTS A. Should be very organized and comprehensive.
III. MAIN SECTION A. Overview of the facilitys operation (approximately 1-2 pages)
1. Brief description of the following:
a. Services provided.
b. Describe the stakeholders.
c. Clients serviced.
e. Personnel used for risk management.
B. Matrix this is the main part of the analysis. This section will include approximately 2 pages of matrices, while the rest will explain the reduction techniques. Photos with appropriate captions should be used for this section (approximately 4-6 pages).
1. Identify TEN (10) specific risks - these should either be risks which you identified, that your contact informed you of, or those that you recognize will be there due to your knowledge from this class. Identify both a financial treatment and a reduction treatment for each risk in your matrix.
2. Your matrix needs classify each risk: severity & frequency (high, medium, low)
3. Use an additional matrix to decide whether to transfer or retain financial risks - indicate how risk will be transferred (i.e. insurance, independent contractor, indemnification clause)
4. Do they use waivers? If they do, provide a copy of the one used. Discuss/decide if it is a well written one. Create a new or altered one that would protect the facility from liability.
5. Use key words for reduction of risks (i.e. supervisory plan, inspections, in-service education plan).
6. Any type of facility will have hundreds of risks associated with it. You are required to cover only 10.
C. Narrative regarding risk reduction procedures. (approximately 3-4 pages)
1. Personnel management
a. Do they utilize miscellaneous personnel policies such as drug testing or anti-smoking? Should they have one if they dont?
b. Discuss the facilitys sexual harassment policy. Create or find (and cite) one if one is not available or does not exist.
2. Discuss their emergency procedures. These should be general procedures for dealing with accidental bodily injuries, natural phenomena, and behavioral problems - not specific first aid procedures.
a. Include a sample accident report form. Is theirs sufficient? If not, provide one that is sufficient (give cite).
D. Compliance with the ADA. (approximately 2-3 pages)
1. Use a checklist either from your textbook or elsewhere to investigate this area. Include the checklist in the appendices section.
2. In this section you will provided a detailed description of what you discovered while completing the Checklist. Photos with appropriate captions are required for this section.
IV. CONCLUSION/SUMMARY (approximately 2-3 pages)
A. Include a comprehensive conclusion or summary; not a couple of short paragraphs thrown together.
V. REFERENCES
A. Must be formatted correctly.
VI. APPENDICES
A. Any forms used, i.e. inspection checklists, participation agreements, rental agreements, lease agreements, waivers, accident report forms, ADA Checklist, etc.
B. The appendices should be discussed in order; first is Appendix A; then Appendix B etc.

ANALYSIS
A. The accurate description of your observations is important. Every section should have an analysis component. Use information from Sports Administration sources (lectures, textbooks, articles) to analyze what you found out and what the impact of your findings has on the facilitys risk management plan.
For example, if the facilitys waiver is poor, you need to comment on this (provide a good alternative). This assignment provides you with an excellent opportunity to demonstrate your knowledge and understanding of the risk management information discussed in this class. You are supposed to be telling the facility:
1. What they are doing,
2. If it is appropriate AND
3. If not, what they should be doing to correct the deficiencies.
B. This paper will be evaluated for thoroughness in identifying risks and planning risk reduction strategies. Err on the side of providing too much detail rather than not enough.
The final paper should be something you could actually use if you were out in the real world. This is supposed to be a Risk Analysis.....NOT a report about Risk Management OR a report about a sport facility.
RISK ANALYSIS PROJECT ASSESSMENT SHEET
1. LENGTH (15 pts) - An appropriate written length should be a minimum of 12 typed and single-spaced pages of TEXT (minus appendixes and references). A minimum of 12 pages earns you at most a C grade. Failure to provide the requisite number of pages will negatively affect your score.
2. ORGANIZATION (15 pts) - There are no specific organization requirements. A detailed table of contents is required. The project will be analyzed for continuity and logical progression. Are there headings dividing the major sections of the paper?
3. STYLE & FORMAT (25 pts) - Appropriate title pages; proper citations; an accurate reference page; miscellaneous formatting (i.e, headings, page numbers, and quotations).
4. QUALITY OF WRITING (20 pts) - A paper of the appropriate length (12-18 pages) is not necessarily an "A" paper. The aspects of quality (neatness, spelling, sentence structure) will be valued more than quantity. The paper should not use the first person tense (I, we, etc.). Verbs should be in the past tense where necessary.
5. CONTENT (75 pts) You begin with 50 points in this section (10 risks @ 5 points each). For each improper classification I will deduct 1 point; for each incomplete financial treatment I will deduct 1 point; for each incomplete reduction treatment I will deduct 1 point; for a poor analysis of the risk I will deduct 1 point. An adequate ADA section will be worth 10 points and you will receive 15 points for an adequate summary/conclusion."
