Create FORM from the following:

1. Hospial name ( from the list : KKH, University, General,.....)

2. Address (street, city, country)

3. Emergency services ( Yes or NO)

4. number of employess

5. Cliccal services (from list choose more than one: Eye , Heart, Childeren, Surgery, Enternal medicine)

6. quality of services (Good, Bad, OK).

7. open date

8. working hours ( for clinics )

9. other separtments ( X-Ray, Lab, Pharmacy,..)

10. Manager name

VALIDATE all fields and make sure if number not allow to enter text.

Save the from in database.

Display TABLE with information of fields saved in the database
