Question: Design a responsive Contact Us form. The Form should contain fields for Name, Email, and Message. The form should have a Contact Us

Design a responsive "Contact Us" form. The Form should contain fields for Name, Email, and Message. The form should have a "Contact Us" banner at the top with simple instructions for the user. On click of the submission button, provide a pop-up notifying the user that the form has been submitted, wait 5 seconds then hide the pop-up automatically and clear the form. Use JavaScript to manage the pop-up. DO NOT use the Javascript alert function. The overall design should be modern and aesthetically pleasing.
