Develop a "Hazard - Barrier - Target Analysis" for each of the three accidents chosen for Discussion Forum 2 above. This analysis should be based on the premise that hazards can be controlled or mitigated by using barriers that are used to protect personnel and equipment.

• For each accident, identify the hazards, targets, and barriers that should have been in place to keep the hazards and targets apart. Be sure to describe the purpose of each barrier and include both physical and management system barriers in the analysis.
• Identify which barriers failed, were unused, or not in place and their impact on the accident.
• Identify the probable causes of the barrier failures for each accident.
• Identify common factors in these criticality accidents.
