• Develop an FMEA for your selected area linked to supply chain events that address four of the seven essential supply chain risks [from module 3]. So, there must be, at a minimum, four rows in the FMEA. If you list at least four risks across the four rows of the FMEA, this requirement will be satisfied [e.g., some failure modes may incorporate multiple risks].
• Include the elements of 1) Event, 2) Failure Mode, 3) Cause of Failure Model, 4) Impact, 5) Severity, 6) Probability, 7) Detectability, 8) Risk Priority Number, 9) Actions / Mitigations, 10) Revised Severity, 11) Revised Probability,12) Revised Detectability, and 13) Revised Risk Priority Number. The student should include their rationale for values inserted in categories 5, 6, 7, 8, 10, 11, 12, and 13.
• The FMEA should have two potential causes for each failure mode.
