Good survey design requires that you "begin with the end in mind" $($Steven Covey$).$ In other words, before you add a question to a survey, you have to consider what you will do with the results. Therefore, each question contained in your survey must include a "Purpose of question statement" $($see examples$).$

The survey results will be used to determine what methods leaders can use to design and implement a strategy initiative.
