Create three field sets: Interviewer Information, Candidate Information, and Candidate's Skills/Presentation. Within the Interviewer Information field set, include fields for the interviewers first name, last name, position, and date of interview. Create the date field using selection lists. Within the Candidate Information field set, include fields for the candidate's first name and last name, a selection list for the interviewer's overall recommendation (Don't Hire, Maybe Hire, and so on), and a comment field. Within the Candidate's Skills/ Presentation field set, include the following selection lists: Intellect, Communication Skills, Business Knowledge, and Computer Skills. Within each selection list, include values such as Average, Above Average, and so on. Use elements for each of the form con- trols. Submit the form to your e-mail address.
