Each student will prepare an Agency Plan for developingaHuman Services & Counseling related program which responds to identified community and/or population needs. The following is a list of required components for the Agency Plan. Additional information may be needed depending on the type of agency chosen. Title of Program Mission statement Values statement Purpose Statement Needs Assessment 1. Demographic data of the region to be served 2. Rationale for the need of the agency Vision and Goals Implementation Plan Community Interaction/Involvement Organizational structure Job descriptions for each employee or anticipated employee named on the organizational structure (e.g., specialization requirements) Advertisement Plan and Materials (e.g., brochure, business cards, etc) Policies and procedures (at least 5) Advisory board information Volunteer information/requirements Itemized Budget Grant/funding information Marketing and fundraising initiatives/Awareness initiatives Legislative information what is your legislature doing for you in this profession?
