Employee #$1$ was using a skid steer to pull logs out of a ravine in the woods, something that was pretty typical for him. This time, the skid steer started to shift sideways and flipped over, rolling down the hill into the ravine. Employee #$1$ was thrown from the cab as it rolled. His head and chest were crushed under the machine, and he was killed from those injuries.
