Begin with your present working (or student) status, briefly described, stating why you are where you are.

State where you intend to go next, and where after that, and after that, etc., as necessary.

For each move you propose to make, indicate what you need to do to make that move (for example, how much education of what kind, experience of what kind).

Provide estimates of how much time each step may require (something of a variation on "where would you like to be in five years? Ten years?").

Finally, for each major milestone in your career plan, indicate what you would plan on doingthat is, how your overall plan might changeshould any of your critical career milestones prove impossible to attain.
