Failure modes and effects analysis (FMEA) could be generated due to various reasons, among others, for a new design or process of new technology. FMEA could assist these activities in completing this new design or process. In the modification to the existing design, assuming that there is an FMEA for the existing design, FMEA could be generated with a focus on the modification to the design process, possible interactions due to the interaction, and field history. FMEA could also be generated on the existing design or process but in a new environment, location or application.
