For this assignment, build a logic model for your program being evaluated. The logic model should be built on updates of the work did for previous formative assignments in the course, as well as guidelines presented in class resources and readings. Your model can appear in any format (text, flow chart, table, etc.), but must include the following components:

The title of the assignment should include the program name and the type of evaluation being conducted. Resources/Inputs.What are the inputs (e.g. financial resources, personnel, coalition partners, facilities, equipment, technology) that will need to carryout your activities that will ultimately lead to meeting the program goal(s) and objective(s)? Briefly describe each of these and the activity/activity to which they are needed/aligned. Activities. What are the specific activities (i.e., tactics) or steps that you will use to achieve your desired objectives? Outputs. What are the immediate, tangible product, services, or other deliverables associated with the activities? What will create? These are typically internal to an organization, and they are used to generate desired outcomes and impacts. Outcomes(short-term). What are the benefits of the outputs to stakeholders and the larger meanings attached to these outputs over the short-term
