Create a title for the survey Create a brief description at the beginning of your survey, including: The purpose of the survey in your own words (do not just copy and paste the research question or objective) A statement that includes all student investigators for the study (first and last names) A statement that includes your group number and group name. An estimated time for survey completion (think about how long it would take to complete if you were actually completing the survey) A statement that says the survey is "for a class project and IRB approval has not been obtained
