Your team has been tasked with the development and implementation of a program that can be implemented throughout the organization or to specific clinical areas to assist in decreasing the harm to patients.

Choose 1 scenario from the choices below and identify 1 appropriate performance improvement model that can address the situation and identify at least 3 performance improvement tools that would allow for you to make improvements. Fictitious data (does not need to be extensive) should be included in order to receive full credit. Make sure to have a plan for implementation and sustainability.
