Have you or a relative ever had a problem with a health insurance claim or getting health services reimbursed?

Hello, I personally don't have any experience with issues related to health insurance or bills. I vaguely remember my father having a considerable number of issues when I was in elementary/middle school because he had been diagosed with lung cancer just as AK Steel in Middletown did the 2006 union lockout, but most of that was kept away from me back then. I know for a fact, though, that he had to go through several appeals processes. Appeals are what I am more confused/curious about. Obviously, patients and providers hope that claims will be submitted and approved without difficulty, but I would like to learn more about how to appeal when they aren't approved, such as what the providers must do or what we as HIM professionals might be expected to do during the process. I imagine that appeals happen fairly frequently and it would be an important topic to understand.
