For this assignment, design a survey that would be sent to customers of the recently centralized release of information department. As part of this process:

1) Identify at least three external customers of the department

2) Determine at least five questions that you would include in your survey to evaluate customer satisfaction

3) Determine at least three other data elements that would be helpful to include in your survey (ex: date of ROI request)

4) Construct your survey, using the guidelines provided for effective survey design on pages 117-119 in your text. Please note somewhere

on your assignment document or in the box below the three external customers your team identified.
