Hi, I need help answering questions for an assignment tonight. It is a Case Formulation/Case Study.
Fictional study : Patient 34 yr Caucasian male with panic disorder with PMH cerebral palsy. He takes care of himself, but lives with parents. He rides a side by side, gets around town. Here is the rubric . I have also copied and paste a sample study for reference. Thank you!

The case formulation assignment is to be 4-5 pages, excluding references.
The five aspects of case formulation must be addressed (recommend using headers to identify).

- Clinical data (include biologic, psychological, and sociocultural data
- Summarize the salient features of the case
- Explanatory
- Important issues that provide insight into the intra-individual and inter-individual aspects of the case
- Prioritize patient problems and interventions
