Fictional study : Patient 34 yr Caucasian male with panic disorder with PMH cerebral palsy. He takes care of himself, but lives with parents. He rides a side by side, gets around town. Here is the rubric . I have also copied and paste a sample study for reference. Thank you!

The case formulation assignment is to be 4-5 pages, excluding references.

The five aspects of case formulation must be addressed (recommend using headers to identify).

1. Integrative
   1. Clinical data (include biologic, psychological, and sociocultural data
   2. Summarize the salient features of the case
2. Explanatory
   1. Important issues that provide insight into the intra-individual and inter-individual aspects of the case
   2. Prioritize patient problems and interventions
   3. Identify criteria for evaluation of intervention effectiveness (symptoms and behaviors to measure = target symptoms and what tools you will use to measure the outcome
3. Prescriptive
   1. Identification of choice of goals (yours and clients)
   2. Medications/therapy and rationale for choice, use EBP to support your choices of medication and dosages
4. Predictive
   1. Prognosis
