Hospice Management System

You are tasked with designing an Entity Relationship Diagram $($ERD$)$ for a hospice

organization. The hospice provides end$-$of$-$life care for patients with terminal illnesses,

focusing on comfort and quality of life. The organization requires a system for managing

various aspects of patient treatment and staff interactions. It wishes to store information

on all persons at the hospice including their TRN$,$ Name $($First$,$ Middle, Last$),$ title,

Date$\_$of$\_$Birth, Gender and Contact$\_$Info. In particular they wish to distinguish particular

information on different persons such as patients $($admission date, diagnosis, primary care

giver$),$ visitors $($relationship to the patient$)$ and staff $($date employed, role, salary$).$

Patients are assigned to rooms but the company would only like to track the person

presently using the room. They can choose to have a private room or a roommate. Each

room has a room number, type, cost and bed number. They also need to capture

information about drugs administered to patients. Attributes such as medication$\_$ID$,$

name of the drug, type $($generic$,$ real$),$ manufacturer as well as the dosage and frequency.

Medication must be administered by a staff member and the date and time it was

administered needs to be recorded.

The Hospice also needs to record the treatment given to the patients. Each patient must be

treated by one or more staff members $($only doctors and nurses can provide treatment$).$

Details about the type of treatment, date, start and end time of treatment needs also to be

stored.

Finally, all staff members report to a senior member in their rank who is the supervisor.

The Hospice only needs to record the current supervisor.

$1.$ Create an Entity Relationship Diagram $($ERD$)$ to show the conceptual design of

the relational database.

$2.$ Use the ERD to create a set of normalized tables for the database design.
