I have a table for Provider Information. Each provider can 0 or more affiliations and employers, and need to be tied to a client. Each of these (affiliation, employer, client) is a facility type. To reduce redundancy, I created a FACILITY table that holds the common information (name, address, phone). Please refer to the database diagram (I did correct the name for FACILITY_CLIENT). My issue the way it is currently set up. I have it affiliation, employer, client tied to the FACILITY table but the attributes are not facility specific, they are provider specific and will be a disaster if left as is. I need a way to connect affiliation, employer, client to the provider. Do I remove the FACILITY table and tie each facility type to the provider?AFFILATION
